Public Interest Disclosure ACT 2002:

This form relates to the Public Interest Disclosures Act 2002 and is provided to assist in making a public interest disclosure to the Council, digitally, but is not compulsory. You are encouraged to complete as much of the form as possible. Disclosures can be disclosed anonymously and will be assessed and considered but may be more difficult to to evaluate.

Devonport City Council encourages the discussion of a public interest disclosure with Council's Public Interest Disclosure Officer in the first instance. Please ensure that you understand your rights and responsibilities under the Public Interest Disclosure Act 2002 before you fill out and submit this form.

Please note that under Section 87(1) of the Public Interest disclosures Act 2002, it is an offence for a person to knowingly provide false information under the Act to certain officers (including the Ombudsman) with the intention that it be acted on as a disclosed matter. The provision provides for a maximum penalty of 240 penalty units or two year's imprisonment, or both.
